Complete each section of the application and press the "Submit Training Application" button at the bottom of this form. A confirmation will be sent to you prior to the beginning of the course. Required Fields are in bold.

Course Registration

Registration Policy: Space is reserved on a first-come, first-serve basis, upon receipt of full payment until course is full.

Important Note: The below courses will be delivered via video teleconference (VTC) or  Elluminate Live! (ELive) web conferencing software. Assignments, quizzes, tests and other activities will be delivered and submitted online throughout the week. Participants need access to a reliable Internet connection, a printer and a valid e-mail address in order to participate in this class.
